ADVERTISEMENT FOR BIDS

The Scott County Board of Commissioners will receive sealed bids for the construction of a bridge replacement on Main Street over Stucker Ditch until 3:00 P.M. local time on July 19th, 2023 at the Scott County Auditor’s Office, 1 E McClain Ave, Scottsburg, IN 47170. Bids may be delivered in person or by mail and shall be addressed to the Scott County Board of Commissioners. All bids will be publicly opened at the regularly scheduled Commissioner’s meeting at 5:00 P.M. on July 19th, 2023, at the Scott County Board of Commissioners Office, 1 E McClain Ave, Scottsburg, IN 47170. Any bids received after the designated time will be returned unopened.

THE PROPOSED CONSTRUCTION CONSISTS OF CONSTRUCTING A CLEAR SPAN (120 FOOT – 0 INCH SPAN STEEL BEAM) BRIDGE (SCOTT COUNTY BRIDGE 105 ON MAIN STREET OVER STUCKER DITCH), TWO NEW END BENTS, ROADWAY APPROACH WORK, SITE GRADING, AND DEMOLITION OF THE EXISTING STRUCTURE ACCORDING TO THE

PLANS AND SPECIFICATIONS.

The Contract Documents, including plans and specifications, are on file for public inspection and may be procured for bidding purposes at the Scott County Highway Department Office, 1101 S Main St, Scottsburg, IN 47170 by payment of a non-refundable plan fee of one hundred and twenty-five dollars ($125.00) for each set of bid documents. Plans and specifications will be available for pickup only at the Scott County Highway Department Office. A check for the plan fee shall be made payable to “Civilcon, Inc.”. All questions regarding the project must be directed to Madison Lauer at (812) 280-8360 (mlauer@civilcondsb.com) or Andrew Boswell (aboswell@civilcondsb.com).

Proposals shall be properly and completely executed on the proposal form furnished to bidders. A certified check, bank draft, or a satisfactory bid bond executed by the Bidder and a surety company in an amount equal to ten percent (10%) of the bid shall be submitted with each bid. The Contractor awarded the work will be required to furnish an acceptable Surety Bond in the amount of one hundred percent (100%) of the contract price. As part of the bid, bidders must demonstrate past experience in bridge construction and provide references for five (5) previously completed projects similar in nature.

The Scott County Board of Commissioners reserves the right to reject any and all bids or to waive any informalities in the bids.

The Scott County Board of Commissioners will hold a public hearing on the following ordinance on Wednesday, July 5, 2023 at 5 p.m. in the Scott County Courthouse.

WHEREAS, the Scott County Health Department is to provide various health services, including, but not limited to testing, certificates and immunization; and,

WHEREAS, the Scott County Board of Health is authorized to establish certain fees and charges for the vital records, testing and immunization services; and,

WHEREAS, the Scott County Board of Health desires to increase the fees charged by the Scott County Health Department for such testing and immunization, effective as of July 1, 2023

NOW, THEREFORE, be it ordained by the County of Scott, Indiana, that the Scott County Board of Health and the Scott County Health Department be, and hereby are, authorized to charge the following fees for services offered by the Scott County Health Department:

Vital Records

Birth Certificate: $10/copy

Death Certificate: $15/copy

Affidavit/Name Change: $20

Sanitation and Septic

Septic Installer Annual Permit Fee: $50 per installer

New Onsite Sewage System Application and Permit: $100

Site Inspection Letter: $40

Partial Onsite Sewage System Repair Permit: $40

Nursing

Immunizations

$5 administration fee (underinsured)

$25 flu shot (uninsured)

Tuberculin Skin Test: $15

Shot Record Copy: FREE

HIV/Hepatitis/STD Testing: $20

Be it further ordained by the Scott County, Indiana, Board of Commissioners that the fees as set forth in this Ordinance are to be effective as of July 1, 2023.
