Pursuant to I.C. 29-1-7-7, notice is hereby given that Steven Trapp was on the 30th day of November, 2017, appointed Personal Representative of the Estate of Murrel R. Trapp, deceased, who died testate on September 28, 2017.
All persons having claims against this estate, whether or not now due, must file the claim in the office of the clerk of this court within three (3) months from the date of first publication of this notice, or within nine (9) months after the decedent’s death, whichever is earlier, or the claims will be forever barred.

PUBLIC HEARING NOTICE

On or about February 9, 2018, the Town of Crothersville intends to apply to the Indiana Office of Community and Rural Affairs for a grant from the State Community Development Block Grant (CDBG) Wastewater/Drinking Water Program. This program is funded by Title 1 of the federal Housing and Community Development Act of 1974, as amended. These funds are to be used for community development project that will include the following activities: replacement of Seymour Road pump station and force main; installation of storm sewer, storm sewer discharge pipe, manholes, storm inlets, and appropriate appurtenances all within the Seymour Road/Cindy Lane/Walnut Street area. The total amount of CDBG funds to be requested is $550,000. The amount of CDBG funds proposed to be used for activities that will benefit low- and moderate-income persons is $281,875. The Applicant also proposes to expend an estimated $144,272 in non-CDBG funds on the project. These non-CDBG funds will be potentially derived from the following sources: Water Utility Operating and Sewer Utility Operating funds.
The Town of Crothersville will hold a public hearing on Tuesday January 2, 2017, at 6:00 p.m. in the Crothersville Town Hall, 111 E. Howard Street, Crothersville, Indiana to provide interested parties an opportunity to express their views on the proposed federally funded CDBG funded project. Persons with disabilities or non-English speaking persons who wish to attend the public hearing and need assistance should contact Office of the Clerk-Treasurer, Town Hall, 111 E. Howard Street, Indiana 47229, 812-793-2311 not later than December 28, 2017. Every effort will be made to make reasonable accommodations for these persons.
Information related to this project will be available for review prior to the public hearing as of January 2, 2018, at the Office of the Clerk-Treasurer, Town Hall, 111 E. Howard Street, Indiana 47229, 812-793-2311 during normal office hours. Interested citizens are invited to provide comments regarding these issues either at the public hearing or by prior written statement. REQUEST FOR PROPOSAL/STATEMENT OF QUALIFICATIONS TOWN OF CROTHERSVILLE WASTEWATER IMPROVEMENTS GRANT PROJECT
In order to assure compliance with the Indiana Office of Community and Rural Affairs (IOCRA) and related requirements regarding competitive negotiation of professional services, the Town of Crothersville, Indiana is seeking Requests for Statements of Qualifications for the provision of engineering services relating to the Wastewater System Improvements Project. This project will be supported in part or in whole by Community Development Block Grant (CDBG) funds through a Wastewater/Drinking Water Program grant funded by the Indiana Office of Community and Rural Affairs.
Description of Project/Scope of Services:
The services will include all necessary services for design and inspection for the recommended improvements based on issues impacting the Seymour Road Pump Station and surrounding area. The recommended improvements include:
•Replacement of the Seymour Road Pump/lift Station
•Replacement of 650 LF of 4″ force main sewer
•Lining 2 manholes
•Installation of 650 LFT of storm sewer and 380 LFT of storm sewer discharge pipe
•Installation of 8 manholes
•Installation of 8 inlets
•Move water lines and laterals as a result of the installation of new pipes
•Restoration of road
•Restoration of yards
Type of Contract
The Town of Crothersville will execute a cost reimbursement type of contract for these services that is contingent on the final commitment of grant funding. All services will be stated on the cost reimbursement basis (hourly rate and maximum hours fixed). The awarded contract will have a not to exceed clause that will limit the allowable fees charged in connection with these services through close out.

The Indiana Office of Community and Rural Affairs (IOCRA) has a 10% MBE/WBE goal for all projects funded with Community Development Block Grant funds.
Rate of Proposals/Qualifications
The proposal must include sufficient information regarding qualifications and determine that the architect/engineer is qualified and experienced in similar projects. Do not include a proposed fee with the proposal, as this is a qualification based selection process. The proposal should include the following:
1. A description of expertise, experience and resources directly relevant and available for the proposed project.
2. A list of similar projects previously completed.
3. A list of references.
4. Resumes of professional staff members that will work on this project.
5. Name of person to be in charge of project.
6. Description of scope of services as per Description of Services Needed
7. A project time line.
The proposal shall also provide the following information:
Name, title, address and telephone number of individuals with authority to negotiate and bind the proposer contractually, and who may be contacted during the period of proposal evaluation.
Award of Contract
Proposal Evaluation criteria shall include:
I. Specialized experience or technical expertise of the organization and its personnel in connection with the scope of services to be provided and complexity of the project. (30 pts)
2.Capacity of the organization to perform the work within time limitations, taking into consideration the current planned workload of the firm. (30 pts)
3.Past record of performance on contracts, including quality of work, timeliness and cost control. (25 pts)
4. Familiarity of the organization with this type of project or problems applicable to the project. (15 pts)
If you are interested in providing the required services, please note that four (4) copies of the Statement of Qualification from each prospective organization must be received by the Town of Crothersville, 111 E. Howard Street, Crothersville, IN 47229 no later than 4:00 p.m. local time on January 22, 2018. The proposal must be submitted in writing in a sealed envelope labeled “Town of Crothersville Wastewater System Improvements Proposal”. Statement of Qualifications received after this time will be returned unopened. Each Statement of Qualification will be reviewed for completeness and clarity according to the above criteria.
The Town of Crothersville may or may not negotiate the fee schedule with one or more offers. The town reserves the right to reject any and/or all proposals. The town is an Equal Opportunity Employer.
