NOTICE TO BIDDERS
Notice is hereby given that Jackson County, Indiana acting by and through its Board of Commissioners, will receive sealed bids at the Office of the Auditor, Jackson County Courthouse, Brownstown, Indiana, until 4:00 p.m. on Monday, March 19, for the 2018 Resurfacing Program.
At the hour of 9 a.m. local time on Tuesday, March 20, 2018, all bids for this work will be opened and read aloud in the Jackson County Annex
Bids shall be unit price for all work called for in the contract documents and submitted on the forms prescribed including and submitting on the forms prescribed including the State Board of Accounts For No. 96 (Revised 2009).
The bidder must file with their sealed bid, a certified check or bid bond in the amount equal to ten (10%) percent of the total bid, payable to the Jackson County Board of Commissioners. The above check or bid bond is to ensure the execution of the contract on which such bid is made.
The contractor to whom the work is awarded will be required to furnish, before commencing work, a performance, maintenance and payment bond in the amount equal to the bid price of the contract awarded to said contractor, and certificates of all insurance required by specifications.

JACKSON COUNTY WATER UTILITY, INC.
2018 ANNUAL
MEMBERSHIP MEETING
This is to inform members of Jackson County Water Utility, Inc. of the upcoming annual membership meeting. The meeting will be held at the Brownstown Central High School cafeteria on Monday, March 12, 2018 at 7:00 p.m. The meeting is held each year for the purpose of electing three (3) members to the Board of Directors, to hear reports of the manager, president and other officers of the corporation and to conduct any and all other business which may properly come before the meeting.
The election process is as follows:
A member of the corporation must nominate another member of the corporation as a candidate for election to the Board of Directors held at the annual meeting of the corporation.
The person so nominated must be a member in good standing and must reside in the district up for election that year.
The nominating member shall provide their name and address and the name and address of the person nominated by them in writing to the Election Committee of the Corporation no later than Monday February 5, 2018. The nomination shall be mailed or delivered to the “Election Committee”, 1119 West Spring Street, Brownstown, Indiana 47220.
The Election Committee shall determine the qualification of the nominating member and the person nominated. These persons so nominated shall be qualified to the ballot for Director by Monday March 5, 2018.
No nomination for Director shall be taken from the floor at the annual meeting. Each membership shall have one vote. (Example: If a membership is listed with both husband and wife’s names, either one may cast a ballot but not both.) The Districts and the current Directors up for election this year are: District 1 (Salt Creek Township), Clayton Beard; District 2 (Hamilton Township), Gary Wente; and District 3 (Redding Township and the North one-half of Washington Township), Robert Akin.

LEGAL NOTICE
TOWN OF CROTHERSVILLE, INDIANA
NOTICE TO BIDDERS
2018 CCMG PROJECT IMPROVEMENTS
Sealed bids will be received by the TOWN OF CROTHERSVILLE, acting by and through the TOWN BOARD (hereinafter referred to as “the Owner”), at 111 WEST HOWARD STREET, CROTHERSVILLE, IN 47229 until 6:00 PM local time, FEBRUARY 6, 2018, for the construction of CCMG PROJECT IMPROVEMENTS for the TOWN OF CROTHERSVILLE.
Bids received by the time and date specified will then be publicly opened and read aloud.
THIS PROJECT CONSISTS OF A LUMP SUM BID FOR BITUMINOUS PATCHING, STRUCTURAL OVERLAY, AND OTHER MISCELLANEOUS AND RELATED ITEMS, AT FOURTEEN SEPARATE LOCATIONS.
After review and award of a contract, the Owner will provide a “Notice to Proceed” to the Contractor. Contractor will then have 10 working days after said “Notice to Proceed” to commence construction and 90 days total to achieve substantial completion and an additional 30 days to complete all work for this project, for a total of 130 days after issuance of “Notice to Proceed” to project closeout. Substantial completion date is therefore anticipated to be July 12, 2018 and final completion date is therefore anticipated to be August 11, 2018, based on a Notice to Proceed and Contract approval by April 3, 2018. Contractor will be assessed $300.00 a day liquidated damages for any work remaining over and above either of the specified contract completion dates. Payment to be made on a standard monthly claim basis, with 10% retainage, and said retainage may be held up to three months after the successful completion of this contract. There is no retainage reduction for this project.
Copies of the Contract Documents are on file for review in the Office of the Owner (TOWN OF CROTHERSVILLE – Clerk Treasurer, 111 WEST HOWARD, CROTHERSVILLE, IN 47229). Access to an ftp site with the Contract Documents in PDF format is available from the Engineer (FPBH, Inc., 72 HENRY STREET, P. O. Box 47, North Vernon, Indiana, 47265, 812-346-2045) for a non-refundable fee of $50.00 per set. Printed copies of the Contract Documents are available for a non-refundable fee of $50.00 per set. Additional sets may be purchased for a non-refundable fee of $25.00 per set. Partial sets will not be available. Only those plan holders registered through the Engineer will be allowed to submit a bid for the project.
There is no pre bid conference scheduled for this project.
Bids must be submitted on the forms in the Contract Documents and other conditions therein described must be met. Each bid must be enclosed in a sealed envelope, clearly marked TOWN OF CROTHERSVILLE – CCMG PROJECT IMPROVEMENTS on the face of the envelope and display the name and address of the bidder. Each bid must be accompanied by a Bid Bond or Certified Check in a sum equal to 10% of the amount of the bid unless otherwise specified, and a completed Non-Collusion Affidavit. Bid prices must be firm for a period of ninety (90) days from the bid opening date. Should a successful bidder withdraw his bid, or fail to execute a satisfactory contract within ten (10) days after notice of acceptance of his bid, the owner may declare the Bid Security forfeited as liquidated damages, not as penalty. The successful bidder shall furnish a Performance and Labor and Materials Payment Bond in an amount equal to one hundred percent
(100%) of the contract sum with an approved surety company. Said bond shall remain in full force and effect for a period of one (1) year after date of final acceptance of the work.
The TOWN OF CROTHERSVILLE, reserves the right to accept or reject any bid and to waive any or all formalities.

The meeting will be held at the Brownstown Central High School cafeteria on Monday, March 12, 2018 at 7:00 p.m. The meeting is held each year for the purpose of electing three (3) members to the Board of Directors, to hear reports of the manager, president and other officers of the corporation and to conduct any and all other business which may properly come before the meeting.
The election process is as follows:
A member of the corporation must nominate another member of the corporation as a candidate for election to the Board of Directors held at the annual meeting of the corporation.
The person so nominated must be a member in good standing and must reside in the district up for election that year.
The nominating member shall provide their name and address and the name and address of the person nominated by them in writing to the Election Committee of the Corporation no later than Monday February 5, 2018. The nomination shall be mailed or delivered to the “Election Committee”, 1119 West Spring Street, Brownstown, Indiana 47220.
The Election Committee shall determine the qualification of the nominating member and the person nominated. These persons so nominated shall be qualified to the ballot for Director by Monday March 5, 2018.
No nomination for Director shall be taken from the floor at the annual meeting. Each membership shall have one vote. (Example: If a membership is listed with both husband and wife’s names, either one may cast a ballot but not both.) The Districts and the current Directors up for election this year are: District 1 (Salt Creek Township), Clayton Beard; District 2 (Hamilton Township), Gary Wente; and District 3 (Redding Township and the North one-half of Washington Township), Robert Akin.

On or about January 18, 2018 the IHCDA will authorize the Town of Crothersville to submit a request to HUD the administering agency for the release of Community Development Block Grant funds under Title I of the Housing and Community Development Act of 1974, as amended, to undertake a project known as:
Project title: Town of Crothersville Owner Occupied Rehab Program
Purpose/Scope of Project: Rehabilitation of owner-occupied housing units
Estimated funding: $120,004
Project location if applicable: Scattered Sites throughout the Town of Crothersville
The activities proposed are categorically excluded under HUD regulations at 24 CFR Part 58 from National Environmental Policy Act requirements. An Environmental Review Record (ERR) that documents the environmental determinations for this project is on file at the IHCDA, 30 South Meridian St., Suite 1000, Indianapolis, IN 46204 can be examined or copied weekdays 8 A.M. to 5 P.M.
PUBLIC COMMENTS
Any individual, group, or agency may submit written comments on the ERR to IHCDA, 30 South Meridian St., Suite 1000, Indianapolis, IN 46204. All comments received January 17, 2018 will be considered by the IHCDA prior to authorizing submission of a request for release of funds.
RELEASE OF FUNDS
The IHCDA certifies to HUD that Jacob Sipe in his capacity as Executive Director consents to accept the jurisdiction of the Federal Courts if an action is brought to enforce responsibilities in relation to the environmental review process and that these responsibilities have been satisfied. HUD’s approval of the certification satisfies its responsibilities under NEPA and related laws and authorities, and allows Town of Crothersville to use Program funds.
OBJECTIONS TO RELEASE OF FUNDS
HUD will accept objections to its release of funds and the IHCDA’s certification for a period of fifteen days following the anticipated submission date or its actual receipt of the request (whichever is later) only if they are on one of the following bases: (a) the certification was not executed by the Certifying Officer of the IHCDA; (b) the IHCDA has omitted a step or failed to make a decision or finding required by HUD regulations at 24 CFR Part 58; (c) Town of Crothersville has committed funds or incurred costs not authorized by 24 CFR Part 58 before approval of a release of funds by HUD; or (d) another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental quality. Objections must be prepared and submitted in accordance with the required procedures (24 CFR Part 58) and shall be addressed to HUD, Office of Community Planning and Development, ATTN: John Dorgan, Director of Community, Planning and Development, Minton Capehart Federal Building, 575 North Pennsylvania Street, Room 655, Indianapolis, IN 46204. On or about February 9, 2018, the Town of Crothersville intends to apply to the Indiana Office of Community and Rural Affairs for a grant from the State Community Development Block Grant (CDBG) Wastewater/Drinking Water Program. This program is funded by Title 1 of the federal Housing and Community Development Act of 1974, as amended. These funds are to be used for community development project that will include the following activities: replacement of Seymour Road pump station and force main; installation of storm sewer, storm sewer discharge pipe, manholes, storm inlets, and appropriate appurtenances all within the Seymour Road/Cindy Lane/Walnut Street area. The total amount of CDBG funds to be requested is $550,000. The amount of CDBG funds proposed to be used for activities that will benefit low- and moderate-income persons is $281,875. The Applicant also proposes to expend an estimated $144,272 in non-CDBG funds on the project. These non-CDBG funds will be potentially derived from the following sources: Water Utility Operating and Sewer Utility Operating funds.
The Town of Crothersville will hold a public hearing on Tuesday January 2, 2017, at 6:00 p.m. in the Crothersville Town Hall, 111 E. Howard Street, Crothersville, Indiana to provide interested parties an opportunity to express their views on the proposed federally funded CDBG funded project. REQUEST FOR PROPOSAL/STATEMENT OF QUALIFICATIONS TOWN OF CROTHERSVILLE WASTEWATER IMPROVEMENTS GRANT PROJECT
In order to assure compliance with the Indiana Office of Community and Rural Affairs (IOCRA) and related requirements regarding competitive negotiation of professional services, the Town of Crothersville, Indiana is seeking Requests for Statements of Qualifications for the provision of engineering services relating to the Wastewater System Improvements Project. This project will be supported in part or in whole by Community Development Block Grant (CDBG) funds through a Wastewater/Drinking Water Program grant funded by the Indiana Office of Community and Rural Affairs.
Description of Project/Scope of Services:
The services will include all necessary services for design and inspection for the recommended improvements based on issues impacting the Seymour Road Pump Station and surrounding area. The recommended improvements include:
•Replacement of the Seymour Road Pump/lift Station
•Replacement of 650 LF of 4″ force main sewer
•Lining 2 manholes
•Installation of 650 LFT of storm sewer and 380 LFT of storm sewer discharge pipe
•Installation of 8 manholes
•Installation of 8 inlets
•Move water lines and laterals as a result of the installation of new pipes
•Restoration of road
•Restoration of yards
Type of Contract
The Town of Crothersville will execute a cost reimbursement type of contract for these services that is contingent on the final commitment of grant funding. All services will be stated on the cost reimbursement basis (hourly rate and maximum hours fixed). The awarded contract will have a not to exceed clause that will limit the allowable fees charged in connection with these services through close out.

The proposal must include sufficient information regarding qualifications and determine that the architect/engineer is qualified and experienced in similar projects. Do not include a proposed fee with the proposal, as this is a qualification based selection process. The proposal should include the following:
1. A description of expertise, experience and resources directly relevant and available for the proposed project.
2. A list of similar projects previously completed.
3. A list of references.
4. Resumes of professional staff members that will work on this project.
5. Name of person to be in charge of project.
6. Description of scope of services as per Description of Services Needed
7. A project time line.
The proposal shall also provide the following information:
Name, title, address and telephone number of individuals with authority to negotiate and bind the proposer contractually, and who may be contacted during the period of proposal evaluation.
Award of Contract
Proposal Evaluation criteria shall include:
I. Specialized experience or technical expertise of the organization and its personnel in connection with the scope of services to be provided and complexity of the project. (30 pts)
2.Capacity of the organization to perform the work within time limitations, taking into consideration the current planned workload of the firm. (30 pts)
3.Past record of performance on contracts, including quality of work, timeliness and cost control. (25 pts)
4. Familiarity of the organization with this type of project or problems applicable to the project. (15 pts)
